Our wellness statement management system was built with your safety in mind.
How It Works
The day before an event, each player/parent will need to attest that they are not knowingly putting others at risk by participating.
Players/Parents will be asked from selected questions such as:
Within the past 24 hours have you had a fever, abnormal cough, shortness of breath, body aches, sore throat, or loss of taste & smell?
Have you knowingly been in close contact with anyone who is known to have or be symptomatic of the Covid19 virus within the last 14 days?
